By enrolling my child into PAK ‘Summer Camping Trip’ at Lake Monroe Campground for July 13th & 14th, 2021, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages which may occur as a result of my child’s participation in any campground event and/or activity. This release is intended to discharge in advance Judah (Pride Academy), it’s directors, executive director, employees and volunteers from liability, even if that liability may arise out of perceived negligence on the part of any person mentioned above. It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks for the children listed below.

As parent/legal guardian I understand I remain fully responsible for any legal responsibility which may result from any personal actions taken by the named child below. By signing I am stating that I understand that some of the listed events will take place away from school grounds and at Lake Monroe Campground that my child will be under the supervision of Pride Academy staff at all times. I understand that all transportation will be provided by Judah (Pride Academy) and/or Miller Transportation.

I further consent to Judah (Pride Academy) taking and publishing photographs and/or videos of the listed child for the purpose of promotion in materials including, but not limited to: social media sites, posters, flyers and newsletters. I understand that since participation in PAK ‘Fall Camping Trip’ it is understood that Judah (Pride Academy) will provide no medical insurance for such treatment, and that the cost will be the parent’s expense.

I give consent for my child(ren)

to participate in all activities of PAK ‘Summer Camping Trip’ at Lake Monroe Campground and I execute the above liability release on their behalf.

I have read and understood this waiver and release of liability for and agree to all of its terms and conditions.
